Summary
A Scaffolding Company (Training Customer) quotes a job and designs a scaffold and as part of that will develop a Site Specific Safety Plan (SSSP) of all stages expected.
The SSSP includes information for a Safe Work Method Statement SWMS across all stages and activities required.
Hazard identifications are done on site and action plans expected / encountered on any stage. This is effectively a questionnaire that may be revisited and added to / questions retired as the job progresses.
Before starting the job a site inspection may be done to revise the SSSP and SWMS and identify any site hazard actions required by the builder
Documents to be available anytime on the job
Stages of work then proceed - Any activity may be multiple days and may require specific skills
Skill and equipment requirements (eg need a attached crane and operator, or need 2x working at height trained persons)
Roster of available persons and Vehicles
Identify vehicles / persons for each day
Assign Site Leading Hand for each day
Every day before doing anything on a worksite
Check for hazards
Identified hazards may update the SWMS and may create a builders action
eg:
new open Pit - avoid area A + Request barriers from builder
digging completed area B - no longer machine hazards
Do a toolbox talk every day covering the latest SWMS
End of toolbox talk every attendee to confirm attendance

Address, Builder (customer), External contact roles on site, internal contact roles on site
SSSP = Project Plan = One Site Specific Safety Plan SSSP of all activities
Scheduled Activities (eg inspect, deliveries, erections, handover certificate, 30day inspections, Adjustments, Remove) - each may take multiple days
Each activity has
a component in the Safe Work Method Statement
Certifications required (eg 2 persons with working at heights)
Assets required (if any) - eg vehicles ?
SWMS for each stage of work (delivery, erection, inspection, change, removal etc) =
Updated with found hazards up to daily and used for toolbox talk
Unexpected Activities may be added at any time
History Reports to show also unexpected activities
Downloadable documents or link to builders documents

Document Definitions
• Safe. Work Method Statement (SWMS) * - sections for each stages of work
• Risk Assessments * - Daily on site - new risks identified - mitigation risk actions - may close the scaffold - site manager - scaffold not usable until fixed. Inspection found problems - can lock gate and tag out scaffold then wait until rectified and re- inspect and open the scaffold. Might have to make a change and record what happened and leading hand updated. Might need to add this to the swims… Then might need to make it inactive when no longer a hazard
• Site-Specific Safety Plans * - list of activities (including risks identified and resolved) across the life of the scaffold.
Requested by Safe work when on site (schedule of actual and planned activities including toolbox talks that did happen changes that did happen and what is expected.) Incidents etc as well - eg scaffolder has found builders are making changes without reference to scaffolding company.
• Diagrams of Build/Construction Plans (Mud Map) * - Image or PDF - Change management Process - image uploaded (normally from screenshot they have modified) then mud map gets updated and the mud map updated and leading hand informed of changes.
• Permits and Licenses * statutory - List required. Other (example working at heights) - RTO deliver these - then leading hand will require that qualification if scaffolding above a certain height (4M). ie Job specific requirement for leading hand (or at least one person on the job)
• Builders Action Plan * Assigned task for remediation / investigation …
• Job Safety Analysis (JSA) * Scaffolding related events across lots of jobs - days with no injuries / incidents. not a document - a report.
Walkaround to add to the SWIMS of hazards to add to the toolbox talk - can review and update each day.
• Toolbox Talk * daily review of swims with persons working that day
• Evacuation Plan * PDF or link to builders plan.
• Incident Reporting (Hazards, incidents, accidents) * - including injuries ( affects job safety analysis report) and non injury
• Handover Certificate * strapped to inside of stairwell - how to achieve this with the builder…
