Questionaire
Overview
A questionaire is used to create a list of questions that are then grouped together in aĀ Questionaire Group
The questionairre has a header area where you can put instructions or notes and some limitations for the questions.
Below the header area is the area that questions are entered - and they appear in the question list as they are saved. Ā You canĀ Browse TestsĀ to review.

How to create questionaires.
- Provide a name for the questionairre
- Decide if the questions are to be limited to a type (useful for scoring)
- If they are to be Numeric, Scale 1 or Scale 2 then you will need to enter the scale details
- Enter the version of the questions - history of versions ensures your answers always are related to the question asked - not the last saved question text
- Decide if multiple tests can be carried out by the same person
- Save

- Text - allows text entry
- Numeric - limits the field to numbers

- Enum - both letters and numbers can be entered in the answer

- Scale1 - creates a simple scale from 1 to the value you enter up to 9
- Scale2 - allows you to enter a scale from 1 to 9 and enter the text for the short end and long end of the scale.

- You can enter the options that can be chosen (not for text or scale questions)
- Select the question in lower LHS of screen
- Enter the question option on RHS and an optional score for that answer
- Save - then clear then enter the next one
Scoring and Recommendations Tab
Used for questionaires that are to be scored similar to a test. You can do simple or complex formulas for scoring and set a cutoff score for a pass/fail result. Each score range can have an associated recommendation and web address
Results analysis tab
This tab provides a list of the completed questionairres by all persons who have answered the questions involved.
Fields on the Screen
Allows entry of multiple types of questions
- Text - text answer
- Numeric - numeric answer - with limits imposed or not as required
- Binary - yes or no
- Enum - A telephone or otherwise defined number
- Scale - a scale from one value to anther (eg 1 to 5)
- Scale2 - a manually entered scale For each question general notes can be entered, option values (if appropriate to the type of question) can be entered to limit selection and have scoring values associated. Scoring recommendations allows adjustment of the scoring results used for testing to determine cutoff results, formula used and can point a particular code if complex requirements exist. Results from a Question can be viewed across all questionaires it has been used in.
